We all agree that those little questionnaire pages that the school gives our children to bring home are the cutest ever, right? You guys loved our Dad Questionnaire for Father’s Day so much (record number of downloads in the first 24 hours), that I had to create a back-to-school questionnaire.
This adorable, free printable back-to-school questionnaire has a few, hand-drawn pictures for them to color (my 15-year-old drew them, by the way). Then, have them (or you, if they are too little), fill in the blanks, in their own words. This would be perfect for the first week of school too, so teachers, this one is for you to print as well!
Have them answer questions like:
- What is your favorite lunch?
- What does your teacher always say?
- How much does your backpack weigh?
- What does your classroom smell like?
- What is your favorite thing about school?
They can use crayons, markers, colored pencils, or if you’re feeling extra brave, watercolors! Don’t worry if they make a mistake. You can just print out another one!
The best part about this back-to-school questionnaire is that it’s a free downloadable file, so you can print off as many as you need. You can even print them off every year and watch your children’s creativity and answers change.
You will love this special keepsake. Don’t forget to add the year to it so you can look back at it for years to come. Hopefully, if your kids are anything like mine, they will come up with some funny answers that will make you giggle.
Once you have finished your back-to-school questionnaire, if you post a photo on social media, we’d love for you to tag @thesupermomlife and encourage others to join in the fun!
Free printable back-to-school questionnaire
This back-to-school questionnaire is easy to download. Just click free back-to-school questionnaire and enter your name and email address (don’t worry, we hate spam as much as you do).
You’ll immediately receive an email with a link to the printable. I recommend printing it immediately and also saving it to your computer so you can print it out again on a later day.
If you love this back-to-school questionnaire, make sure to check out our other free printables. We have some for all occasions, with lots more coming soon!

What a great idea for younger kids, it will help them get excited and ready for the new term ahead and like you say, it’s a lovely keepsake too. Mich x
This is so cool. I think my niece and nephews would love it. Will let them answer it.
This is so nice! I am so glad that there are such printables. Something that kids can have as activity sheets and memory when they grow older.
Cute! I’ll have to use this for my little cousins!!
Ok. not only is this so cute, but I adore that your 15 year old created the drawings kids can colour! They’re awesome! And how fun that you got to work together on this sweet project <3. Thank you for sharing it with us!
How amazing! I love activities like this. The kids would love it for sure 😍
What a fun way to get kids engaged in starting the school year. It’s also a wonderful memento to have as they get older too.
