We all agree that those little questionnaire pages that the school gives our children to bring home are the cutest ever, right? You guys loved our Dad Questionnaire for Father’s Day so much (record number of downloads in the first 24 hours), that I had to create a back-to-school questionnaire.
This adorable, free printable back-to-school questionnaire has a few, hand-drawn pictures for them to color (my 15-year-old drew them, by the way). Then, have them (or you, if they are too little), fill in the blanks, in their own words. This would be perfect for the first week of school too, so teachers, this one is for you to print as well!
Have them answer questions like:
- What is your favorite lunch?
- What does your teacher always say?
- How much does your backpack weigh?
- What does your classroom smell like?
- What is your favorite thing about school?
They can use crayons, markers, colored pencils, or if you’re feeling extra brave, watercolors! Don’t worry if they make a mistake. You can just print out another one!
The best part about this back-to-school questionnaire is that it’s a free downloadable file, so you can print off as many as you need. You can even print them off every year and watch your children’s creativity and answers change.
You will love this special keepsake. Don’t forget to add the year to it so you can look back at it for years to come. Hopefully, if your kids are anything like mine, they will come up with some funny answers that will make you giggle.

What a fun way to get kids engaged in starting the school year. It’s also a wonderful memento to have as they get older too.
