We all agree that those little questionnaire pages that the school gives our children to bring home are the cutest ever, right? You guys loved our Dad Questionnaire for Father’s Day and Back to School Questionnaire so much (record number of downloads in the first 24 hours) that I had to create a Valentine’s Day questionnaire for you too.
This adorable, printable Valentine’s Day questionnaire has a few hand-drawn pictures for them to color (my 16-year-old drew them, by the way). Then, have them (or you, if they are too little) fill in the blanks in their own words. This would also be a great activity for school, so teachers, this one is for you to print as well!
Have them answer questions like:
- What is your favorite candy?
- Who is your Valentine?
- How many Valentine’s Day cards will you receive this year?
- How old are you going to be when you get married?
- What is your favorite thing about Valentine’s Day?
They can use crayons, markers, colored pencils, or watercolors if you’re feeling extra brave! Don’t worry if they make a mistake. You can print out another one!
The best part about this Valentine’s Day questionnaire is that it’s a downloadable file, so you can print off as many as you need. You can print them off yearly and watch your children’s creativity and answers change.
You will love this special keepsake. Don’t forget to add the year to it so you can look back at it. Hopefully, if your kids are anything like mine, they will come up with funny answers that will make you giggle.
Once you have finished your Valentine’s Day questionnaire, if you post a photo on social media, we’d love for you to tag @thesupermomlife and encourage others to join in the fun!

I really love this idea. I may need some of these myself. I’d love to have them printed out.
I love the idea and I really love that kids can be involved, this is awesome! Great idea.
This is such a cute Valentine’s Day idea for the kids. I think it’s super adorable.
This is a cool homemade Valentine. It would be fun to fill out.
This is such a cute printable, it would be lovely to keep and remember in years to come all the things they loved!
Such a super cute printable for kids. I wonder how they would answer the “how old will you be when you get married” question. When I was a kid I thought I’d get married at like 20 and have a kid at like 22. That seemed a lot older to me at the time, lol.
