We had such a great response to our Back to School Questionnaire that I thought creating one for the first day of school would be fun.
This adorable printable first-day-of-school questionnaire has a few hand-drawn pictures for them to color (my 15-year-old drew them, by the way). Then, have them (or you, if they are too little) fill in the blanks in their own words and draw a picture of themselves in the little box. This would be perfect to complete at school, so teachers, this one is also for you to print!
They can use crayons, markers, or colored pencils to color it in! Don’t worry if they make a mistake. You can print out another one!
The best part about this first-day-of-school questionnaire is that it’s a downloadable file, so you can print off as many as you need. You can print them off yearly and watch your children’s creativity and answers change.
