We had such a great response to our Back to School Questionnaire that I thought creating one for the first day of school would be fun.
This adorable printable first-day-of-school questionnaire has a few hand-drawn pictures for them to color (my 15-year-old drew them, by the way). Then, have them (or you, if they are too little) fill in the blanks in their own words and draw a picture of themselves in the little box. This would be perfect to complete at school, so teachers, this one is also for you to print!
They can use crayons, markers, or colored pencils to color it in! Don’t worry if they make a mistake. You can print out another one!

That is a cute printable. My son had something similar when he was younger.
This is so cute! I remember doing pages like this when my kids were younger, and I miss doing them. So fun!
I might adapt it for bigger kids. So many teachers play the thumb on ball or two-lies-and-a-truth game that they get sick of it, this might be a nice switch.
This is such a memory maker if you manage to do this every year! I would’ve loved to have this from my own childhood.
