We all agree that those little questionnaire pages that the school gives our children to bring home are the cutest ever, right? You guys loved our Dad Questionnaire for Father’s Day and Back to School Questionnaire so much (record number of downloads in the first 24 hours) that I had to create a Valentine’s Day questionnaire for you too.
This adorable, printable Valentine’s Day questionnaire has a few hand-drawn pictures for them to color (my 16-year-old drew them, by the way). Then, have them (or you, if they are too little) fill in the blanks in their own words. This would also be a great activity for school, so teachers, this one is for you to print as well!
Have them answer questions like:
- What is your favorite candy?
- Who is your Valentine?
- How many Valentine’s Day cards will you receive this year?
- How old are you going to be when you get married?
- What is your favorite thing about Valentine’s Day?
They can use crayons, markers, colored pencils, or watercolors if you’re feeling extra brave! Don’t worry if they make a mistake. You can print out another one!
The best part about this Valentine’s Day questionnaire is that it’s a downloadable file, so you can print off as many as you need. You can print them off yearly and watch your children’s creativity and answers change.
You will love this special keepsake. Don’t forget to add the year to it so you can look back at it. Hopefully, if your kids are anything like mine, they will come up with funny answers that will make you giggle.
Once you have finished your Valentine’s Day questionnaire, if you post a photo on social media, we’d love for you to tag @thesupermomlife and encourage others to join in the fun!

That is a fun printable for kids. I would have fun answering it myself. 🙂
This is fun and a nice way to keep them busy. Will ask my girls to answer this. Thank you for sharing the questionnaire.
This is so very cute. My darlings will love it. I’ll print these for my nieces and nephews.
This is such a cute and thoughtful activity to try! Your 16yo has skills!
This is so adorable! I think its a great way to make valentines day still about love but also fun for kids. Plus, it a great keepsake for them to see what they wrote when they get older.
I love the idea. Way to make a child engaged and understand valentine’s with the questions on the printable.
Super super cute. I haven’t sent my Valentine’s Day cards yet, so I’ll print these for my nieces and nephews.
